1. All accepted presentations of the 85th Annual Meeting are certified to be presented at the meeting, but Invited lectures and Requested talks can be re-presented at the 86th Annual Meeting. In this case, please leave this form and use the Application form for Re-presentation.
2. All presentations will be presented at on-line. If you cannot present at your Office, School or Home, you can present at the "Live-stream presentation venue" (Tokyo). If you wish to use the venue, please select "NG-Net" or "NG-Zoom" on the `OnlineAvailable' question (the 3rd question) and write your circumstances and any requests in the `OnlineCircumstances' box (the 4th question).
